Transcript Request for current students requesting a transcript for a scholarship or school not accepting electronic transcripts on Naviance.
Graduate Transcript Request Form - Complete this form and allow up to 48 hours to complete your request.
Student Service Hours Verification Form - Complete this form after completing your service hours (time sensitive).
Service Learning Site Approval Form - If completing hours at a non-approved site, please submit this form to Ms. Rivera prior to completing your service hours.
Work Permit - Work permits are now completed only by the student, parent, and employer. LHS no longer signs or provides work permits.
Learner's Permit School Attendance Verification - Print, complete, get signed by parent, and submit to counseling secretary
Scholarship Nomination Form - Used to nominate yourself for a scholarship listed in the College and Career newsletter. The scholarship will state whether a nomination form is required or not.
Schedule Change Request - Used for students who wish to change a class on their schedule within the first few days of school. Must meet the county approved reasons for schedule changes.
